Pumping can lead to permanent enlargement, but it's not clear-cut. Frequent pumping can cause scar tissue to form, increasing the girth of the penis. According to Gary Griffin, this scar tissue is harmless. This is doubtless responsible for the "permanent enlargement" many pumpers claim.
A fellow at the BME website says he broke his suspensory ligament (ouch!) so that his he can pull penis about two inches farther out of his body. This didn't happen from pumping, but it's an example of permanent alteration. (I wouldn't want to do it, though.)
