Lottameat is right -- you need to lube your cock all over to keep your skin soft. I use baby oil, plus udder balm around the opening of the tube, to help with irritation where the tube contacts my body.
I use a foreskin tube when I've had a good pump and have a big, bloated foreskin. First you push the acrylic ball down inside the opening of your foreskin. It's function is to keep the head of your cock from getting sucked into the foreskin tube.
Then, holding your foreskin closed over the ball, you put the foreskin tube over the top and pump. this is easier to do with an electric pump, but I can do it OK with a manual pump too. When you apply vacuum, your foreskin will squish out around the ball and up into the foreskin tube. The sensation of this happening is unreal.
After your foreskin stretches so far it fills the tube, you can stretch it further by adding another ball. One foreskin freak buddy of mine uses 3 balls. The foreskin gets pinched fairly tight between the flared opening of the tube and ball, restricting the circulation, so you need to take frequent breaks, say every 5-10 minutes and thoroughly massage in between.
