You put a vacuum around your balls and the pressure inside your
capillaries becomes higher than the pressure inside the pump
cylinder. That causes extra fluid to move from your bloodstream
into the spaces surrounding the testicles, and the spaces gradually
fill up with blood, distending your scrotum with fluid. It also
causes microhemorrhages inside the testicles themselves, and if you
apply enough pressure, it can cause the testicles to swell, potentially
causing significant injury.
For up to 24 hours or so after the scrotum has been inflated this way,
the fluid will gradually be reabsorbed into the lymphatic circulation
and return into the blood from there.
