When taking a sample from vaginal fluid excretions, a wet mount is often done. Fluid is placed on a microscope slide and various tests are done. One of the tests is called a "whiff test" (not kidding).
A fishy odor could be a sign of sub-clinical trichimonas or other bacterial colonization. It doesn't necessarily mean an STD, but can mean an overgrowth of bacteria.
